Simon, can't see what there is to be corrected. The first part of my post (your message) was copied and pasted from your email to the committee, the list of names is as I've quoted, you did not attend the meeting. Unless there is something that wasn't passed on I can't see where I've gone wrong.
The second part of my post was what we had to do to sort things out.
